Continuing the Commitment: 51²č¹Ż Primary Care Earns National Recognition for the 9th Year in a Row

51²č¹Ż Primary Care Center has once again been nationally recognized as a Patient-Centered Medical Home by the National Committee for Quality Assurance (NCQA). This marks the ninth consecutive year that 51²č¹Ż has received this honor, which is awarded to primary care practices that consistently provide coordinated, personalized care with a strong focus on the patient experience.

This recognition means patients can trust they’re receiving care that is not only high quality but also designed around their unique needs.  ā€œThe Patient-Centered Medical Home is a model of care that puts patients at the forefront of care. Our Primary Care Center locations build strong relationships between patients and their clinical care teams,ā€ shared Mayra Perez, DO, medical director at UT Health Hill Country. ā€œAchieving the NCQA PCMH recognition year after year reaffirms our commitment to delivering to continuous quality improvement and enhancing access to a patient-centered approach to care.ā€

All 51²č¹Żā€™s primary care locations were awarded the Patient-Centered Medical Home recognition in 2025. 

Now one year since opening, UT Health at Kyle Seale Parkway has been recognized as a Patient-Centered Medical Home (PCMH), a testament to its commitment to high-quality, coordinated care. Richel Avery, MD, associate chief medical officer at UT Health at Kyle Seale Parkway, shared, ā€œOver the past year, our team has focused on delivering safe, accessible and comprehensive care to our patients. With the PCMH recognition, we’re proud to continue building on this foundation—prioritizing coordination, integration and a personalized approach to every patient we serve.ā€

Delivering Seamless, Coordinated Care

At its core, the Patient-Centered Medical Home model is about building long-term relationships between patients and their care teams. It means every person receives personalized care that’s coordinated across all their health needs—from routine checkups to managing chronic conditions like diabetes or high blood pressure. The goal is to keep people healthier by working together, catching problems early and avoiding unnecessary hospital visits.

51²č¹Ż uses technology to make that kind of teamwork easier. With a shared electronic medical record, doctors, nurses, care managers and other providers can all stay connected and informed about each patient’s care. This ensures that the right care happens at the right time, without delays or duplication.

Care managers work closely with patients who have ongoing health concerns, helping them stay on track with medications, appointments and treatment plans. In addition to doctors and nurses, the care team may include behavioral health consultants, pharmacists and community health workers who support patients with everything from mental health care to health education and guidance.

ā€œThis model improves the quality of care patients receive—from better chronic condition management to earlier disease detection. But much of the care we provide also happens outside the exam room,ā€ said Jessica Kitchen, MD, medical director at UT Health Westover Hills. ā€œMedical assistants and nurses respond quickly to questions through MyChart, care managers notify us of hospital visits and community health workers offer personalized education and support to improve health outcomes.ā€

Improving Access to Care

Getting care when you need it is also a major part of what makes this model successful. 51²č¹Ż offers same-day and next-day appointments for urgent needs at all of its primary care locations. Many clinics also offer extended and weekend hours to fit busy schedules. Through MyChart, patients can easily send a message to their care team, schedule appointments, view test results or request medication refills—all from a phone or computer. 51²č¹Ż Primary Care Center patients can even use MyChart to request eVisits and one-click video visits to address minor, non-emergency medical issues that don't require in-person testing or physical examinations.

Being recognized as a Patient-Centered Medical Home for nine years in a row shows just how committed 51²č¹Ż is to doing things the right way. Every doctor, nurse and staff member is focused on making care more personal, accessible and effective for every patient.
